Steps for Handling a Food Service Civil Rights Complaint

 

If a parent or guardian calls or comes to any of the school offices with a civil rights complaint, the Director of Food Services has all the necessary Civil Rights complaint forms provided by the  USDA. The Administrator in charge will contact the parent and gather all needed information from the parent or guardian and will determine what the next step will be. If a formal complaint needs to be made to the USDA, the Administrator in Charge of that case will be available help the parent or guardian fill out the Civil Rights Complaint form and submit them to the USDA. USDA, Director, Office of Adjudication, 1400 Independence Avenue, SW, Washington, D.C. 20250-9410 or call toll free (866) 632-9992 (Voice). Civil Rights Non-Discrimination Statement In accordance with Federal civil rights law and U.S. Department of Agriculture (USDA) civil rights regulations and policies, this institution is prohibited from discriminating on the basis of race, color, national origin, sex (including gender identity and sexual orientation), disability, age, or reprisal or retaliation for prior civil rights activity. The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in suffucient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation.
